Contract
Wildwood Dachshunds guarantees this Dog to be free of
communicable disease for a period of 7 days from the Date
of Sale stated on this contract. While in our care, the Dog
has been on a routing health schedule. The Dog has received
scheduled vaccinations which will aid in the prevention of
such diseases as parvovirus, distemper, etc. The Dog has
also been on a regular de-worming schedule. Strongid T
liquid wormer has been given as directed by our
veterinarian beginning at the age of 2 weeks. However,
Wildwood Dachshunds does not guarantee against worms or
parasites. Wildwood Dachshunds will guarantee against the
listed viruses for 7 days from the Date of Sale. (A Dog
Shot History listing viruses will be provided to you at
time of Sale) and for 6 months against hereditary defects
if the following requirements are met:
1. Animal MUST be vet checked within 72 hours of purchase
date. Please keep a receipt or
report.
2. Both a licensed Veterinarian and I
MUST be notified at
the first sign of illness in Dog.
3 The animal returned, or if death should occur, a signed
and dated statement from a licensed Veterinarian showing
hereditary defect or cause of death.
When the requirements are met in full, I agree to replace
the animal with one of same sex and equal value. I do not
guarantee cash refund nor am I responsible for any vet
bills that might incur. Buyer hereby waives any action or
cause of action against Wildwood Dachshunds and/or Denise
Smith for any damage incurred as a result of this sale.
